Must be available all shifts.

SUMMARY:

Provides direct and indirect resident patient care activities under the direction of a Registered Nurse or Licensed Practical Nurse.

Assists residents with activities of daily living, provides for personal care, comfort, and assists in the maintenance of a safe and clean environment for an assigned group of residents.

DUTIES AND RESPONSIBILITIES:

1. Provides care with activities of daily lifting which may include bed bath, oral hygiene, combing hair, back care, back rubs, dressing residents.

Other duties include changing bed linen, cleaning over bed table and bedside stand, straightening room, and other general care as necessary throughout the day.

2. Assists Staff Nurse and/or Licensed Nurse in treatment procedures.

3. Provides general nursing care such as positioning residents; lifting and turning resident; applying/utilizing special equipment; assisting in use of bedpan, urinal or commode; ambulating the resident using a gait belt.

4. Takes and records temperature, pulse, respiration, weight, height, blood pressure and intake-output measurements.

5. Adheres to Policies and Procedures of the facility and the Department of Nursing.

6. Checks for reddened areas or skin breakdown and reports to RN or LPN.

7. Provides clean linen and clothing.

Removes all bed spreads from beds, folds neatly and places at foot of beds.

Makes beds and/or assists residents with making their beds as necessary.

8. Restrains residents properly when ordered.

9. Accompanies residents to appointments as directed.

10. Provides reality orientation in daily care.

11. Prepares residents for meals, serves and removes food trays, and assists with meals or feeds residents.

12. Distributes drinking water and other nourishments to residents.

13. Performs general care activities for resident's isolation.

14. Answers residents' call lights, anticipates residents' needs, and makes rounds to assigned residents.

15. Assists residents with handling and care of clothing and other personal property (including dentures, glasses, contact lenses, hearing aids, prosthetic devices).

16. Transports residents to and from various departments as requested.

17. Reports (and when appropriate, records) any changes observed in condition or behavior of residents and unusual incidents.

18. Participates in and contributes to Nursing Care Plans.

19. Provides post-mortem care and assists in transporting bodies to the morgue.

20. Assists in maintaining a safe and clean environment by cleaning Utility Rooms, cleaning equipment and preparing for disinfection or sterilization, reporting equipment that is out of order, reporting other problems to the charge nurse, appropriate handling of clean and dirty linen and stores supplies on the unit.

21. Establishes and maintain interpersonal relationships with residents, family members, and other facility personnel, while assuring confidentiality of resident information.

22. Attends in-service education program, as assigned, to learn new treatments, procedures, developmental skills, etc.

QUALIFICATIONS:
Education Requirements: C.N.A Certification

English Language Skills:

Must be able to read, write and speak the English language in an understandable manner.

Physical Demands: Must be in good general health and demonstrate emotional stability.

Must be able to lift up to and in excess of 25 pounds throughout the workday.

Sit, stand, bend, lift and move intermittently during working hours.

Working Environment:

Works in well lighted/ventilated areas.

Subject to hostile and emotionally upset residents, family members, etc.
